When you scheduled your appointment, our administrative team should provide you our New Patient paperwork. Kindly fill out the paperwork before your first visit with Dr. Franco. You can also book an appointment with Franco Dental submitting your initial information in our Appointment Section
Dental Insurance
If you have dental insurance, please make sure to bring your insurance card so that we can work directly with your insurance company to determine your benefits and arrange payment. If you provide your insurance information before your first visit, we can complete a complimentary benefit verification prior to your visit so that you know exactly what services will be covered and what your financial responsibility will be.
No Dental Insurance?
For patients who do not have dental insurance we offer the Franco Dental Preventive Plans. This is not an insurance plan. It is an in-house discount plan we developed to alleviate the cost of dentistry for our patients.
Print Out
We are in network with several major insurance companies and will submit claims to all insurances.
We ask that new patients print out the appropriate form below and bring it to your first appointment
